01How do I schedule an appointment?
You can schedule an appointment at Elite Medical Group by calling our office at (248) 918-0800 or booking online through our website.
Please bring your insurance card, a valid photo ID, and a list of your current medications to your appointment to ensure accurate and efficient care.
02Does Elite Medical Group accept my insurance?
Elite Medical Group partners with many insurance plans, including Medicare, Medicaid, PPOs, HMOs, and others. To ensure coverage, we recommend checking directly with your insurance provider to confirm our physicians are in-network, or you may contact our office for assistance.
Please bring your insurance card and a valid photo ID to each visit for verification. If you do not have insurance, payment will be due at the time of service. Because benefits vary by plan, we encourage you to review your policy details with your insurance carrier or employer.
03Do I need a Primary Care Physician (PCP)?
At Elite Medical Group, we strongly encourage patients to establish care with a primary care physician. Having a PCP ensures your health is managed in a coordinated way, with our providers guiding your care and collaborating with specialists when necessary.
In some cases, your insurance plan may require you to designate a PCP. We recommend reviewing your policy details or contacting your insurance provider directly—their phone number can usually be found on the back of your insurance card.
04What hospitals is Elite Medical Group affiliated with?
Elite Medical Group is proud to be affiliated with the following hospitals:

Our affiliations allow us to coordinate your care seamlessly if hospital services or specialized care are needed.
05How can I transfer or obtain my medical records?
If your medical records have not yet been sent to Elite Medical Group, we can request them on your behalf. To protect your privacy, a signed and dated Medical Records Release (HIPAA) form is required to release your records.
If you’re relocating or changing providers, we can forward your records to your new physician upon request. This process may take up to 30 days. There is no charge for records sent directly to another healthcare provider, but fees may apply for personal, legal, or insurance purposes.
06How do I get a referral to a specialist?
At Elite Medical Group, your primary care physician coordinates your care and will provide referrals to specialists when needed. Because referral rules vary by insurance plan, we recommend confirming the process with your insurance provider to ensure coverage.
07What should I do in an emergency?
For life-threatening emergencies, call 911 immediately. For urgent but non-life-threatening issues, contact our office during business hours or visit the nearest urgent care or emergency room after hours.
